    Background

    Employed households experience time poverty, which refers to feeling overwhelmed because of the struggle to balance work and life. Time poverty is subjectively perceived as a lack of personal free time. In Japan, long working hours and societal expectations regarding the division of work and family roles may influence the perception of time poverty. This issue is of significant concern, as it can impact individuals’ rest time and work productivity. However, there is currently no standardized measurement method to assess time poverty appropriately in the Japanese context. The lack of such a method challenges establishing a foundation for developing effective support strategies. Given this background, this study aimed to quantify time poverty among employed households by developing a Japanese version of the Perceived Time Poverty Scale and examining its reliability and validity.

    Methods

    In developing the Japanese version of the Perceived Time Poverty Scale, cultural adaptations were made in addition to the standard translation and back-translation procedures. Through discussions with researchers and translation experts, terms with differing scopes of interpretation in the Japanese context were revised, and expressions were adjusted to reflect the intended concepts better. The data for this study were collected through Wave 2 of the longitudinal survey, the Hama Study, conducted over a five-year period from 2022 to 2027. This survey randomly selected 10,000 employed households residing in Yokohama, Japan. Participants completed the Japanese version of the Perceived Time Poverty Scale developed in this study, along with the well-being scale, the Kessler Screening Scale for Psychological Distress, and the Japanese Short-Form UCLA Loneliness Scale. Exploratory and confirmatory factor analyses were conducted to evaluate the scale structure. Internal consistency was assessed using Cronbach’s alpha and McDonald’s omega coefficients. Furthermore, correlations between the Japanese version of the Perceived Time Poverty Scale and the other scales were examined to evaluate the structural validity of the scale.

    Results

    Data from 1,979 respondents who participated in the Wave 2 online survey were analyzed. The scale demonstrated high reliability, with a Cronbach’s alpha coefficient 0.90 (95% CI: 0.89–0.91). Exploratory factor analysis confirmed a single-factor structure and confirmatory factor analysis supported this structure with fit indexes (CFI =  0.957, TLI =  0.929, RMSEA =  0.136, SRMR =  0.035). Perceived time poverty was negatively correlated with sleep time and leisure time, and positively correlated with childcare time. Furthermore, perceived time poverty showed significant correlations with well-being, psychological distress, social isolation, and job satisfaction, confirming the validity of the developed scale.

    Conclusion

    The Japanese version of the Perceived Time Poverty Scale is a reliable tool with a certain degree of validity for assessing time poverty in Japan. This scale enables individuals and households to recognize time poverty as a modern form of poverty. Furthermore, businesses and local governments can utilize it as an indicator in practical settings, such as improving work environments, implementing childcare support programs, and promoting community health. Future longitudinal studies are needed to further validate the scale, including addressing issues related to model fit.

    Purpose

    This study examines the Food for Specified Health Uses (FOSHU) system in Japan, which features a unique certification logo (FOSHU seal) not present in other functional food systems and whose influence on consumer purchase intention remains unclear. Specifically, it aims to (1) examine the FOSHU seal’s influence on consumer purchase intention, (2) compare the effects of the FOSHU seal and health claims and (3) assess the impact of consumers’ perception of FOSHU, their FOSHU purchasing experience, and the perception of their gastrointestinal condition.

    Design/methodology/approach

    Data were collected from Japanese respondents through a questionnaire survey using conjoint measurements. Ordered multinomial logistic regression analysis was employed, with FOSHU seal, health claims, sugar-free and extra quantity labels as independent variables and ordered purchase intention as the dependent variable.

    Findings

    The results indicated that the FOSHU seal on functional food products positively influences consumer purchase intention, with health claims exerting a greater influence than the FOSHU seal. Additionally, purchase intention is positively affected when consumers perceive the FOSHU seal as a mark of authenticity.

    Originality/value

    This study is novel in two respects. First, it extends brand research, which has traditionally focussed on corporate and product brands, to certification marks, demonstrating their effect on purchase intention. Second, creating similar logos for functional foods beyond those for specified health uses in Japan or in similar systems in other countries could improve purchase intentions.

    Abstract

    Aim

    This study observed changes in respiratory rate measurement (RRM) and identified barriers and challenges in clinical practice that influence healthcare worker behaviour, aiming to improve RRM in a hospital setting.

    Design

    An observational study was conducted.

    Methods

    We observed and analysed changes in the behaviour of healthcare workers at a hospital where multi‐nudges were introduced to improve RRM.

    Results

    We checked respiration rate using electronic data and discovered that the original measurement rates were low. Measurement rates rapidly increased after posters were added. Barriers such as time constraints and measurement equipment were also noted.

    Conclusion

    RRM was found to be effective in promoting behavioural economics in medical settings. The results show that incorporating behavioural science principles into medical interventions has the potential to positively change behaviour.

    Implications for the Profession and/or Patient Care

    By increasing nurses' awareness of respiratory rate measurement and addressing barriers to it, the measurement rate of respiratory rate can also increase, leading to more accurate patient evaluations and triage.

    The proportion of respiratory rate measurements leading to rapid response system (RRS) calls was low.

    In 2022, the Japan Society for Healthcare Administration (JSHA) launched the Young Members Association (YMA) for exchange and study among young members, presentation of their achievements, and career development. As an activity of the YMA, we conducted the first hospital tour at Toho University Medical Center Ohashi Hospital. Thirteen individuals, including graduate students, medical professionals, and researchers, participated in the tour, comprising lectures by the administrative director, deputy director, and others, a hospital tour, and group discussions. Ohashi Hospital downsized from 433 to 319 beds with relocation, and efforts and design innovations were made to achieve high turnover and unit price. Consequently, the tour participants deepened their learning on architecture and management. Simultaneously, they gained new perspectives and suggestions for future activities by exchanging with other participants from different backgrounds. The YMA will continue to plan hospital tours and other events to promote the development of the JSHA.

    Introduction

    Many developed countries including Japan are experiencing declining birth rates, particularly in urban areas. A gap between the planned number of children and the actual number of children exists, that is attributed to various factors such as: childcare leave and employment policies, childcare services, financial support, husbands’ contributions to household chores and childcare, marriage support, community, and couples’ well-being. Therefore, we propose HAMA study for having a baby, parenting, and marriage life (HAMA = ‘H’aving ‘A’ baby, parenting, and ‘MA’rriage life) in Yokohama (an urban area) to examine these issues.

    Methods and analysis

    In this large-scale cohort study, we will elucidate the actual situation of families and child-rearing in Yokohama, evaluate the current policies and propose future measures to prevent a decline in the birth rate. Overall, 10 000 young married couples (wives aged 20–39 years as of 2022) will be randomly selected, and a survey form will be sent to them annually. They will be followed-up for 5 years to examine the factors associated with the planned number of children, well-being of the couple, childcare support policies, externalisation of housework and childcare, fathers’ participation in housework and childcare, wives’ free time, loneliness and social connectedness, relationship with the spouse and if they are working, questions regarding their work style and work-life balance will also be included. Ultimately, a conceptual model of the planned number of children and associated factors will be developed.

    Ethics and dissemination

    This study has been approved by the Ethics Committee of Yokohama City University (reference number: 2022–10) and will be conducted following appropriate ethical guidelines. Opportunities to withdraw consent to participate in the survey are provided to participants. The results of this survey will be published as research papers in relevant journals and will be reported to the administration of Yokohama city and other agencies.

    Abstract

    Background

    In Japan’s super-aging society, the number of long-term care service providers is increasing, and the quality of care is a matter of concern. One aspect of the quality of care is the user’s quality of life. The questionnaires EQ-5D and WHO-5 are representative indicators of quality of life. Herein, we aimed to measure the quality of life in long-term care service users in Japan and to clarify the relationship between quality of life and the level of care required.

    Methods

    A questionnaire study was conducted in 106 facilities of 22 corporations. In addition to the EQ-5D and WHO-5, sex, age, and the level of care required were assessed by descriptive statistics. Bonferroni’s multiple comparison test was used to analyze each quality of life score, and the differences by sex and age were analyzed multiple regression analyses, with each quality of life score as the objective variable.

    Results

    Of 4647 cases collected, 2830 were analyzed, with no missing data. Both indicators tended to be lower than the general older population. Those scores tended to be higher in females than males (EQ-5D: males, 0.58 ± 0.26; females, 0.60 ± 0.24; P = 0.06 and WHO-5: males, 13.8 ± 5.92; females 14.9 ± 5.70; P < 0.001). In terms of age, those under 65 years old with specific diseases had lower EQ-5D scores than those in other age groups (P < 0.001); however, WHO-5 scores did not differ by age. Multiple regression analysis showed a significant association between the EQ-5D score and level of care required, except for support-required level 1, which tended to worsen as the level of care required increased. Conversely, the WHO-5 score was significantly lower for care need levels 2, 4, and 5.

    Conclusions

    The quality of life of long-term care service users was worse than that of the general older population, it tended to be low among males and those under 65 years old with specific diseases. Furthermore, it gradually decreased as the level of care required increased. It is important to monitor users’ quality of life as a quality indicator of care, to improve and manage it.

    Objectives In this longitudinal study, we examined changes in the geographical distribution of physicians in Japan from 2000 to 2014 by clinical specialty with adjustments for healthcare demand based on population structure. Methods The Japanese population was adjusted for healthcare demand using health expenditure per capita stratified by age and sex. The numbers of physicians per 100 000 demand-adjusted population (DAP) in 2000 and 2014 were calculated for subprefectural regions known as secondary medical areas. Disparities in the geographical distribution of physicians for each specialty were assessed using Gini coefficients. A subgroup analysis was conducted by dividing the regions into four groups according to urban-rural classification and initial physician supply. Results Over the study period, the number of physicians per 100 000 DAP decreased in all specialties assessed (internal medicine: -6.9%, surgery: -26.0%, orthopaedics: -2.1%, obstetrics/gynaecology (per female population): -17.5%) except paediatrics (+33.3%) and anaesthesiology (+21.1%). No reductions in geographical disparity were observed in any of the specialties assessed. Geographical disparity increased substantially in internal medicine, surgery and obstetrics and gynaecology(OB/GYN). Rural areas with lower initial physician supply experienced the highest decreases in physicians per 100 000 DAP for all specialties assessed except paediatrics and anaesthesiology. In contrast, urban areas with lower initial physician supply experienced the lowest decreases in physicians per 100 000 DAP in internal medicine, surgery, orthopaedics and OB/GYN, but the highest increase in anaesthesiology. Conclusion Between 2000 and 2014, the number of physicians per 100 000 DAP in Japan decreased in all specialties assessed except paediatrics and anaesthesiology. There is also a growing urban-rural disparity in physician supply in all specialties assessed except paediatrics. Additional measures may be needed to resolve these issues and improve physician distribution in Japan.

    Objectives: The objective of this study was to longitudinally examine the geographic distribution of physicians in Japan with adjustment for healthcare demand according to changes in population age structure.
    Methods: We examined trends in the number of physicians per 100 000 population in Japan's secondary medical areas (SMAs) from 2000 to 2014. Healthcare demand was adjusted using health expenditure per capita. Trends in the Gini coefficient and the number of SMAs with a low physician supply were analysed. A subgroup analysis was also conducted where SMAs were divided into 4 groups according to urban-rural classification and initial physician supply.
    Results: The time-based changes in the Gini coefficient and the number of SMAs with a low physician supply indicated that the equity in physician distribution had worsened throughout the study period. The number of physicians per 100 000 population had seemingly increased in all groups, with increases of 22.9% and 34.5% in urban groups with higher and lower initial physician supply, respectively. However, after adjusting healthcare demand, physician supply decreased by 1.3% in the former group and increased by 3.5% in the latter group. Decreases were also observed in the rural groups, where the number of physicians decreased by 4.4% in the group with a higher initial physician supply and 7.6% in the group with a lower initial physician supply.
    Conclusions: Although the total number of physicians increased in Japan, demand-adjusted physician supply decreased in recent years in all areas except for urban areas with a lower initial physician supply. In addition, the equity of physician distribution had consistently deteriorated since 2000. The results indicate that failing to adjust healthcare demand will produce misleading results, and that there is a need for major reform of Japan's healthcare system to improve physician distribution.
