    Traditional mouse models for deep vein thrombosis (DVT), frequently utilized in research focused on cancer-associated thrombosis (CAT), reliably induce thrombus formation by obstructing blood flow (BF) in the inferior vena cava (IVC), which does not occur in humans. Therefore, to develop a new DVT model for CAT studies, we implanted an ameroid constrictor (AC), a hygroscopic casein C-shape device, around the IVC and aorta of immunocompromised mice. We evaluated the thrombus 3 and 8 days post-AC implantation and compared it with the traditional model 2 days post-vena cava ligation. The size of each thrombus was measured, and the composition was assessed using histological staining; BF through the IVC was confirmed using ultrasound imaging. The thrombus size variability in the AC and ligation models was equivalent. Compared with thrombi on day 3 post-AC implantation, those on day 8 showed characteristics of human thrombi in the subacute to chronic stage. The BF in the IVC was maintained even on day 8. In summary, the AC model showed reproducibility with no significant difference in thrombus size variability from the traditional ligation model while maintaining the BF of the IVC.

    Tissue factor pathway inhibitor-2 (TFPI2) is a Kunitz-type serine protease inhibitor and an ovarian clear cell carcinoma (CCC) biomarker. TFPI2 is expressed in several cancers and exerts tumor-suppressive effects; however, the role of TFPI2 in the CCC cell phenotype remains unclear. Therefore, in this study, we investigated the function of TFPI2 by establishing a gene knockout (KO) in ES-2 CCC cells and observed the change in phenotypes in vitro and in vivo. TFPI2 KO inhibited ES-2 cell proliferation, increased extracellular matrix protein adhesion, enhanced focal adhesion formation and activated integrin β1 cell surface clustering in vitro, and markedly increased ES-2 tumor growth and dissemination in the peritoneal cavity of a mouse xenograft model. These findings suggest a novel function of TFPI2 expression in suppressing the formation of focal adhesions in CCC cells, potentially by activating integrin β1. This function plays a role in the peritoneal growth characteristics of CCC cells.

    BACKGROUND: Antibody-drug conjugates (ADCs) are attractive targeted agents in anticancer treatment with their unique mechanism of action and reduced toxicity. Little is known about the spectrum of adverse events associated with ADCs, despite tens of clinical trials. METHODS: A systematic review of randomized controlled trials evaluating ADCs efficacy in anticancer treatment was conducted. PubMed, EMBASE, and ClinicalTrial.gov were searched for relevant studies. Meta-analyses assessed the odds ratios (ORs) of 12 treatment-related symptoms and toxicities in ADC-treated patients compared to those receiving other anticancer agents without ADCs. All-grade and high-grade (grade 3 or higher) toxicities were examined. RESULTS: 20 studies involving 10,075 patients were included. Compared with control groups, ADCs were associated with a higher risk for all-grade fatigue (OR, 1.25; 95% CI, 1.08-1.45), anorexia (OR, 1.36 [1.09-1.69]), nausea (OR, 1.46 [1.09-1.97]) and sensory neuropathy (OR, 2.18 [1.27-3.76]) as treatment-related symptoms. Patients treated with ADCs had a significantly lower risk of all-grade febrile neutropenia (OR, 0.46 [0.22-0.96]). Conversely, they had a higher risk of thrombocytopenia (OR, 2.07 [1.00-4.31]), increased alanine aminotransferase (OR, 2.51 [1.84-3.40]), and increased aspartate aminotransferase (OR, 2.83 [2.04-3.93]). Subgroup analysis showed a similar toxicity profile when comparing the solid tumors vs hematologic malignancy groups and the ADC vs ADC plus chemotherapy groups, except for some neurologic and hematologic adverse events. CONCLUSIONS: This comprehensive profile of adverse events associated with ADC-based treatment shows an increase in various types of all-grade treatment-related symptoms and adverse events although no increase in high-grade adverse events was seen.

    Cancer tissues generally have molecular oxygen and serum component deficiencies because of poor vascularization. Recently, we revealed that ICAM1 is strongly activated via lipophagy in ovarian clear cell carcinoma (CCC) cells in response to starvation of long-chain fatty acids and oxygen and confers resistance to apoptosis caused by these harsh conditions. CD69 is a glycoprotein that is synthesized in immune cells and is associated with their activation via cellular signaling pathways. However, the expression and function of CD69 in non-hematological cells is unclear. Here, we report that CD69 is induced in CCC cells as in ICAM1. Mass spectrometry analysis of phosphorylated peptides followed by pathway analysis revealed that CD69 augments CCC cell binding to fibronectin (FN) in association with the phosphorylation of multiple cellular signaling molecules including the focal adhesion pathway. Furthermore, CD69 synthesized in CCC cells could facilitate cell survival because the CD69-FN axis can induce epithelial-mesenchymal transition. Experiments with surgically removed tumor samples revealed that CD69 is predominantly expressed in CCC tumor cells compared with other histological subtypes of epithelial ovarian cancer. Overall, our data suggest that cancer cell-derived CD69 can contribute to CCC progression through FN.

    BACKGROUND: Cervical cancer is the second most common cancer in women and causes more than 250,000 deaths worldwide. Among these, the incidence of cervical adenocarcinomas is increasing. Cervical adenocarcinoma is not only difficult to detect and prevent in the early stages with screening, but it is also resistant to chemotherapy and radiotherapy, and its prognosis worsens significantly as the disease progresses. Furthermore, when recurrence or metastasis is observed, treatment options are limited and there is no curative treatment. Recently, heavy-particle radiotherapy has attracted attention owing to its high tumor control and minimal damage to normal tissues. In addition, heavy particle irradiation is effective for cancer stem cells and hypoxic regions, which are difficult to treat. METHODS: In this study, we cultured cervical adenocarcinoma cell lines (HeLa and HCA-1) in two-dimensional (2D) or three-dimensional (3D) spheroid cultures and evaluated the effects of X-ray and carbon-ion (C-ion) beams. RESULTS: X-ray irradiation decreased the cell viability in a dose-dependent manner in 2D cultures, whereas this effect was attenuated in 3D spheroid cultures. In contrast, C-ion irradiation demonstrated the same antitumor effect in 3D spheroid cultures as in 2D cultures. In 3D spheroid cultures, X-rays and anticancer drugs are attenuated because of hypoxia inside the spheroids. However, the impact of the C-ion beam was almost the same as that of the 2D culture, because heavy-particle irradiation was not affected by hypoxia. CONCLUSION: These results suggest that heavy-particle radiotherapy may be a new therapeutic strategy for overcoming the resistance of cervical adenocarcinoma to treatment.

    Abstract

    Elucidation of spatial interactions between cancer and host cells is important for the development of new therapies against disseminated cancers. The aim of this study is to establish easy and useful method for elucidating spatial interactions. In this study, we developed a practical spatial analysis method using a gel-based embedding system and applied it to a murine model of cancer dissemination. After euthanization, every abdominal organ enclosed in the peritoneum was extracted en bloc. We injected agarose gel into the peritoneal cavities to preserve the spatial locations of the organs, including their metastatic niches, and then produced specimens when the gel had solidified. Preservation of the original spatial localization was confirmed by correlating magnetic resonance imaging results with the sectioned specimens. We examined the effects of spatial localization on cancer hypoxia using immunohistochemical hypoxia markers. Finally, we identified the mRNA expression of the specimens and demonstrated the applicability of spatial genetic analysis. In conclusion, we established a practical method for the in vivo investigation of spatial location-specific biological mechanisms in disseminated cancers. Our method can elucidate dissemination mechanisms, find therapeutic targets, and evaluate cancer therapeutic effects.

    BACKGROUND: Serum starvation and hypoxia (SSH) mimics a stress condition in tumours. We have shown that intercellular adhesion molecule-1 (ICAM-1) protein is synergistically expressed in ovarian clear cell carcinoma (CCC) cells under SSH in response to an insufficient supply of fatty acids (FAs). This ICAM-1 expression is responsible for resistance against the lethal condition, thereby promoting tumour growth. However, the underlying mechanisms that link SSH-driven ICAM1 gene expression to impaired FA supply and its clinical relevance are unclear. METHODS: The underlying mechanisms of how FA deficiency induces ICAM-1 expression in cooperation with hypoxia were analysed in vitro and in vivo. Clinical significance of CCC cell-derived ICAM-1 and the mechanism associated with the transcriptional synergism were also investigated. RESULTS: ICAM-1 expression was mediated through lipophagy-driven lipid droplet degradation, followed by impaired FA-lipid droplet flow. Lipophagy induced ICAM1 expression through stabilisation of NFκB binding to the promoter region via Sam68 and hTERT. Analyses of clinical specimens revealed that expression of ICAM-1 and LC3B, an autophagy marker associated with lipophagy, significantly correlated with poor prognoses of CCC. CONCLUSIONS: The lipophagy-ICAM-1 pathway induced under a tumour-like stress conditions contributes to CCC progression and is a potential therapeutic target for this aggressive cancer type.

    PURPOSE: Diagnosis of breast preneoplastic and neoplastic lesions is difficult due to their similar morphology in breast biopsy specimens. To diagnose these lesions, pathologists perform immunohistochemical analysis and consult with expert breast pathologists. These additional examinations are time-consuming and expensive. Artificial intelligence (AI)-based image analysis has recently improved, and may help in ordinal pathological diagnosis. Here, we showed the significance of machine learning-based image analysis of breast preneoplastic and neoplastic lesions for facilitating high-throughput diagnosis. METHODS: Images were obtained from normal mammary glands, hyperplastic lesions, preneoplastic lesions and neoplastic lesions, such as usual ductal hyperplasia (UDH), columnar cell lesion (CCL), ductal carcinoma in situ (DCIS), and DCIS with comedo necrosis (comedo DCIS) in breast biopsy specimens. The original enhanced convoluted neural network (CNN) system was used for analyzing the pathological images. RESULTS: The AI-based image analysis provided the following area under the curve values (AUC): normal lesion versus DCIS, 0.9902; DCIS versus comedo DCIS, 0.9942; normal lesion versus CCL, 0.9786; and UDH versus DCIS, 1.000. Multiple comparison analysis showed precision and recall scores similar to those of single comparison analysis. Based on the gradient-weighted class activation mapping (Grad-CAM) used to visualize the important regions reflecting the result of CNN analysis, the ratio of stromal tissue in the whole weighted area was significantly higher in UDH and CCL than that in DCIS. CONCLUSIONS: These analyses may provide a more accurate and rapid pathological diagnosis of patients. Moreover, Grad-CAM identifies uncharted important histological characteristics for newer pathological findings and targets of research for understanding diseases.

    Tissue factor pathway inhibitor‑2 (TFPI‑2) is a promising candidate as a serum biomarker of ovarian clear cell carcinoma (OCCC), a lethal histological subtype of epithelial ovarian cancer (EOC). TFPI‑2 is a secreted serine protease inhibitor that suppresses cancer progression through the inhibition of matrix protease activities. Previous studies have also identified TFPI‑2 in the nucleus, and a possible function of nuclear TFPI‑2 as a transcriptional repressor of matrix metalloproteinase‑2 (MMP‑2) was recently demonstrated. We are currently establishing TFPI‑2 as a serum biomarker for OCCC patients; however, TFPI‑2 expression in OCCC tissues has not been previously investigated. In the present study, we examined TFPI‑2 expression and its localization in 11 OCCC cell lines by western blotting and enzyme‑linked immune assay. Four cell lines expressed TFPI‑2 in the nucleus, cytoplasm and culture plate-attached extracellular fraction, while four other cell lines expressed TFPI‑2 only in the extracellular fraction. In the remaining three cell lines, TFPI‑2 was not identified in any fraction. The amount of secreted soluble TFPI‑2 showed similar trends to that of the plate‑attached fraction. We next investigated the expression levels and distribution of TFPI‑2 in surgically resected EOC tissues by immunohistochemistry. In 52 of the 77 (67.5%) OCCC tumors, TFPI‑2 expression was detected in at least one of the nuclear, cytoplasmic and extracellular matrix fractions. In contrast, we did not identify TFPI‑2 in the other EOC subtypes (n=65). TFPI‑2‑positive expression distinguished CCC from the other EOC tissues with a sensitivity of 67.5% and specificity of 100%. Although the inherent tumor suppressor function, statistical analyses failed to demonstrate correlations between TFPI‑2 expression and clinical parameters, including 5‑year overall survival, except for the patient age. In conclusion, we identified TFPI‑2 expression in the nucleus, cytoplasm and extracellular matrix in OCCC tissues. The high specificity of TFPI‑2 may support its use for diagnosis of OCCC in combination with existing markers.

    子宮体癌根治術として、全腹腔鏡下準広汎子宮全摘術+両側付属器摘出術を施行した52例(A群)と、開腹準広汎子宮全摘術+両側付属器摘出術を施行した35例(B群)の治療成績を比較した。その結果、手術時間はA群で有意に長かったが、術中出血量はA群で有意に少なく、入院期間もA群で有意に短かった。合併症はA群26.9%、B群25.7%に認め、両群とも神経因性膀胱が最多であった。A群について執刀件数別に30件以上:C群、10件以上30件未満:D群、10件未満:E群に分け比較したところ、手術時間はD群がE群に比較し有意に短かったが、出血量に3群で差はなかった。合併症はいずれも神経因性膀胱が多く、統計的な差はないがE群に多い傾向があった。なお、神経因性膀胱発症例の動画を振り返り検討したところ、非発症例と比較して手術時間全体が延長傾向にあり、特に膀胱子宮靱帯前層の処理時間が延長傾向にあることが判明した。

    症例は40歳(0妊0産)で、子宮筋腫による過多月経と貧血が進行し全腹腔鏡下子宮全摘出術、両側卵管切除術を行った。手術所見でダグラス窩腹膜に約1cmの青黒色の結節を認め周囲に大網が一部癒着しており、子宮内膜症所見はr-ASRM分類2点であった。病理診断で子宮筋層・漿膜、両側卵管は子宮内膜症所見はなく、子宮筋腫であった。術後、周期的な性器出血や腹痛、臍部の疼痛があり、腟断端の生検で腟断端子宮内膜症の診断を得た。臍部はポート孔に生じた子宮内膜症を疑い生検を行ったが診断は得られなかった。術後、腟断端と臍部に子宮内膜症が生じたと考えてジェノゲスト内服を行い、症状は改善している。

    42歳(2経妊0経産)。不正出血を主訴とした。子宮内膜組織診で類内膜腺癌Grade2であり、骨盤MRI検査・頸部〜骨盤造影CT検査で子宮体癌IB期相当と診断した。体重109kg、BMI 44.8と高度肥満のため、全身麻酔導入後に除圧式固定具のマジックベッド上で上腹部から胸骨前面までの2ヶ所と側腹部から乳房頭側までの4ヶ所をテープで固定し、腹壁を平担にして腹腔鏡手術を行った。エンドクローズを用いてS状結腸の脂肪垂と子宮を牽引することで良好な視野を保ちながら、準広汎子宮全摘・両側付属器摘出・骨盤リンパ節郭清術を施行することができた。病理診断は子宮体癌IIIC1期(pT2N1M0)、組織型は類内膜腺癌と粘液性腺癌の混合癌であり、合併症なく術後6日で退院し、術後18日目より化学療法としてtri-weekly TC療法を6クール施行した。術後10ヵ月現在、無病生存中である。

    80歳女。2型糖尿病、高血圧、腹腔鏡下胆嚢摘出術の既往があり、糖尿病の通院中に便潜血陽性で上行結腸腹側に腫瘤を指摘され受診となった。CEA、CA19-9は正常範囲内、右側腹部に弾性硬で腹壁とともに移動する腫瘤を触知した。造影CTで右下腹部腹腔内に腹壁に接する径6×3cm大の境界明瞭な腫瘤を認めた。造影MRIではT1強調像で等信号、T2強調像で軽度高信号を呈し、大腸内視鏡で径10mm大のポリープを二つ認め、腹壁デスモイドと判断し小開腹で腹壁腫瘤摘出術を行った。右側腹壁の径60×40mm大腫瘤を腹横筋、内腹斜筋の筋層ごと摘出し外腹斜筋腱膜は温存し、腫瘤は筋層でとどまっていた。表面白色でカニみそ色の粥状の内容物を有する比較的薄い壁の嚢胞性腫瘤で、病理所見より内側は表皮に類似した角化重層扁平上皮で裏打ちされ、内側は上皮が剥離消失しており、腹壁由来の類表皮嚢胞(epidermoid cyst)と診断した。
