    The endoscopic endonasal approach has emerged as an effective and minimally invasive technique for the management of tuberculum sellae meningiomas. A 53-year-old woman with progressive visual deterioration due to a tuberculum sellae meningioma extending into the optic canal underwent endoscopic endonasal resection with optic canal decompression. Gross-total removal was achieved using a "French-door" dural opening and angled dissection, resulting in Simpson grade II resection with the preservation of neurovascular structures. Optic canal opening enabled safe tumor removal and significant visual improvement. Reconstruction employed sutured fascia lata grafts and a sphenoid sinus mucosal flap resulted in no postoperative cerebrospinal fluid leakage.

    PURPOSE: Postoperative weight gain following endoscopic endonasal surgery (EES) for craniopharyngiomas remains a challenging complication. Thus, in this study, we aimed to identify risk factors for new or worsened obesity following EES and to explore age-related patterns of postoperative weight change. METHODS: We conducted a retrospective cohort study of 72 patients (26 pediatric and 46 adult) who underwent initial EES for craniopharyngioma at two academic centers. We defined obesity based on body mass index (BMI) in adults and the BMI standard deviation score in pediatric patients. The primary outcome was new or worsened obesity at follow-up. Tumor extension and hypothalamic involvement/damage were assessed using standardized classification systems. Multivariate logistic regression was used to identify independent predictors. RESULTS: New or worsened obesity was observed in 17 patients (23.6%). Retroinfundibular tumor extension (odds ratio [OR]: 9.81; p = 0.002) and postoperative hypothalamic damage (OR: 7.72; p = 0.016) were independently associated with new or worsened obesity. However, surgical factors, including resection and operative complexity, were not significant predictors. Subgroup analyses revealed that a subset of pediatric patients developed early-onset and persistent obesity, whereas adults (particularly young adults) experienced gradual weight gain. CONCLUSION: Retroinfundibular tumor extension and hypothalamic damage are key risk factors for postoperative obesity following EES, whereas aggressive resection itself does not increase the risk. Pediatric patients may develop obesity early and persistently, whereas adults, particularly young adults, are prone to gradual weight gain. Early diagnosis and strategic treatment for high-risk individuals may improve the prognosis for patients with hypothalamic obesity.

    OBJECTIVE: Lesions of the ventral craniovertebral junction are difficult to access owing to their deep location and proximity to critical neurovascular and pharyngeal structures. In this study, we aimed to describe the surgical technique and clinical outcomes of the endoscopic endonasal transnasopharyngeal approach for ventral craniovertebral junction lesions and highlight key considerations regarding approach selection, airway management, and occipitocervical stabilization. METHODS: We retrospectively reviewed 7 patients who underwent the endoscopic endonasal transnasopharyngeal approach for ventral craniovertebral junction lesions. The analysis included preoperative planning for surgical access, intraoperative technique, postoperative management, airway and nutritional strategies, and the need for occipitocervical fixation. One representative case is presented to illustrate key technical steps. RESULTS: Of the 7 patients, 6 had neoplastic lesions and 1 had basilar invagination. Despite a relatively large mean lesion size of 39.4 mm, subtotal or greater resection was achieved in 5 of the 6 tumor cases. Occipitocervical fixation was performed in 2 cases. Two patients underwent prophylactic tracheostomy because of anticipated airway compromise. Of the 5 orally intubated cases, 3 were extubated immediately and 2 by postoperative day 2. Oral feeding resumed by day 10 in 6 cases. No postoperative infections or cerebrospinal fluid leakage occurred. One patient experienced transient velopharyngeal insufficiency, which resolved spontaneously. CONCLUSION: The endoscopic endonasal transnasopharyngeal approach is a safe and effective option for ventral craniovertebral junction lesions when appropriately selected. Careful preoperative evaluation and individualized management of airway and spinal stability are essential for favorable outcomes.

    PURPOSE: Preservation of endocrine function in pediatric patients with craniopharyngioma is crucial. However, few reports of endoscopic endonasal surgery (EES) for pediatric craniopharyngiomas with intending endocrine function preservation exist. This study aimed to identify the outcomes of EES with intending preservation of pituitary stalk in pediatric patients with craniopharyngioma. METHODS: Data from 22 pediatric patients with craniopharyngioma who underwent 35 EESs between 2011 and 2022 were retrospectively assessed. The tumor location (infradiaphragmatic [ID] or supradiaphragmatic [SD]) and endocrine functions were evaluated. The patients were divided into two groups: those with preoperatively preserved some endocrine function (Group P) and already completely lost endocrine function (Group ACL). Morphological preservation of the pituitary stalk (MPPS) and postoperative endocrine functional outcomes were analyzed only in the cases of Group P. Tumor recurrence was evaluated in all patients. RESULTS: In the 35 EESs, the tumors were ID in 17 cases and SD in 18. Twenty-five cases were included in Group P and 10 in Group ACL. In Group P, MPPS was achieved in 18 (72%) cases, and postoperative preservation of some endocrine function was achieved in 14 (77.8%) of the MPPS cases. ID tumor (P < 0.001) and intraoperative MPPS (P < 0.001) were associated with postoperative preservation of some endocrine function. Intraoperative MPPS (P = 0.044) was a significantly associated risk factor for tumor recurrence. CONCLUSIONS: In EES for pediatric craniopharyngiomas, postoperative preservation of some endocrine function is achievable in ID tumors with MPPS. Therefore, preservation of endocrine function should be attempted in cases of ID tumors. However, clinicians should be aware that MPPS increases the risk of tumor recurrence.

    BACKGROUND: The fully endoscopic combined endonasal and supraorbital keyhole approach, which integrates the endoscopic endonasal approach (EEA) and the endoscopic supraorbital keyhole approach (eSKA), is effective for complex suprasellar tumors extending beyond the internal carotid artery (ICA). We detail its application for tuberculum sellae meningiomas with lateral extension. METHOD: Tumor resection is performed via the EEA, with support from the eSKA. This allows for careful management of the optic nerve under direct visualization and enables sufficient tumor resection. CONCLUSION: This combined approach facilitates extensive tumor resection, effective skull base reconstruction, and preservation of surrounding structures.

    Preoperative simulation for endoscopic endonasal approach(EEA)using computed tomography and magnetic resonance imaging evaluates tumor extension and the relationship between adjacent structure(the pituitary stalk, major vessels, and cranial nerves); therefore, preoperative planning of nasal procedure, skull base bony removal, and cranial base reconstruction are possible. Additionally, three-dimensional(3D)fusion image aids surgeons to visualize intraoperative 3D findings. These preoperative simulations are critical to avoid complications and predict pitfalls perioperatively. However, tumor consistency or adhesion with adjacent structure cannot be predicted but is judged perioperatively, which affects the extent of tumor resection. This manuscript describes important points of preoperative simulation for EEA, especially the transplanum-transtuberculum approach for craniopharyngiomas or tuberculum sellae meningiomas, showing some examples in patients.

    ラトケ嚢胞の術後に嚢胞内容液が再貯留をきたし再治療を要する症例をたびたび経験する。著者らは再貯留を防ぐ術式として、蝶形骨洞粘膜を用いた嚢胞-蝶形骨洞包袋化手術(Mucosa coupling法)を施行している。本法は、蝶形骨洞粘膜を用いて嚢胞腔と蝶形骨洞を単洞化させることで、嚢胞内容液を常に蝶形骨洞へドレナージする排出路を確立する方法であるが、正常下垂体組織が蝶形骨洞と同じ空間に存在することとなり、長期的な内分泌学的な安全性については疑問を残していた。また、硬膜再建を行わず手術手技を終了する本法の術後髄液漏リスクについても不明であった。今回、これらについて明らかにするため、術後長期観察しえた24例の内分泌機能と術後髄液漏発生頻度を調査し、従来術式を施行した群(33例)と比較した。その結果、内分泌機能、術後髄液漏発生頻度とも有意な群間差は認めなかった。

    Dermatofibrosarcoma protuberans (DFSP) originates from the dermal layer of the skin; the optimum treatment is an extended marginal resection. We describe a case of DFSP of the scalp with a skull invasive defect that was thoroughly examined pathologically to determine the optimum length of surgical margins. The tumor cells infiltrated up to 26 mm into the dermal tissues, whereas no infiltrating tumor cells were present in the skull, indicating the combination of marginal resection of the dermal tissues and lower of the skull can be a clinically relevant strategy for treatment of DFSP cases with skull invasion.
