Abstract

Background

We previously reported an association between higher phase angle (PhA) and better cognitive performance in older adults with cognitive impairment ¹⁾ . However, whether PhA changes over time in relation to cognitive decline remains unclear.

Methods

Between 2018 and 2022, we recruited 240 older adults (mean age 82.4 ± 6.5 years; mean MMSE 21.9 ± 5.0) who had objective or subjective cognitive impairments over two consecutive years and completed both MMSE and physical assessments. Cognitive function was measured using the Mini‐Mental State Examination (MMSE). Physical assessments included body mass index, total muscle mass, limb skeletal muscle mass, fat percentage, estimated bone mass, and PhA. Linear mixed‐effects models examined changes from the first to second year, adjusting for age, sex, education, and height. One‐year change scores were calculated for all measures, and comparisons were made between participants with Alzheimer disease (AD) and those with mild cognitive impairment (MCI).

Results

MMSE scores declined significantly from year one to year two ( p  < 0.001), as did total and limb skeletal muscle mass (both p  < 0.05) and PhA ( p  < 0.001). Other anthropometric measures (body mass index, fat percentage, visceral fat, and estimated bone mass) remained stable. At baseline, participants with AD had lower MMSE scores (mean difference 7.31, p  < 0.001); however, their one‐year decline (–1.01 points) did not differ significantly from that of MCI participants (–1.68 points, p  =  0.061). In multiple regression analyses, lower education and AD status significantly predicted greater MMSE decline ( p  < 0.05).

Conclusions

Over one year, older adults at risk for cognitive decline showed decreases in MMSE, muscle mass, and PhA, while other anthropometric measures remained unchanged. Although AD participants consistently had lower cognitive scores, their rate of decline was similar to those with MCI. Monitoring both physical and cognitive indicators, particularly PhA, may support early detection of deterioration in older adults with or at risk for AD.

ABSTRACT

Objective

This study employed a symptom network analysis to elucidate the interrelationships among key memory domains assessed by the Wechsler Memory Scale‐Revised (WMS‐R) in adults aged 75 and older with cognitive impairment. Our goal was to identify the central components within the memory network that may serve as promising targets for early and precision interventions.

Methods

A total of 265 participants were recruited from a dementia center in Kyoto, Japan (2018–2022). Four WMS‐R subtests—assessing verbal memory, visual memory, attention and concentration, and delayed recall—were administered. Spearman's rank‐order correlations were used to explore pairwise associations. A sparse network structure was estimated using the EBICglasso regularization method in R, and centrality metrics (Strength, Betweenness, and Closeness) were calculated to determine the relative prominence of each domain. The robustness of the network was evaluated through extensive bootstrap analyzes.

Results

The results revealed strong positive correlations among verbal memory, visual memory, and delayed recall, while attention and concentration were less strongly linked to the other domains. Network analysis consistently identified visual memory and delayed recall as the most central nodes—with the strongest edge observed between these two functions. Bootstrap procedures confirmed the stability and precision of the major edges and the strength centrality metric, although betweenness and closeness indices showed lower stability.

Conclusion

Visual memory and delayed recall emerged as pivotal components in the cognitive network of older adults with impairment, suggesting that interventions focusing on these domains may enhance overall cognitive function.

Background

Detecting life disability is crucial in diagnosing dementia; however, early detection has proven challenging with previous assessment scales. This study focused on an individual's ability to use household appliances as a means of detecting life disability.

Objective

The objectives of this study are threefold: (1) to compare the ability to use household appliances between the non-dementia and dementia groups, (2) to determine whether the level of life disability based on the ability to use appliances is at the level of diagnosed dementia or non-dementia, and (3) to explore the impact of age and gender on the ability to use appliances.

Methods

We selected 13 essential household appliances for elderly individuals and proposed an instrumental activities of daily living (IADL) assessment tool to evaluate their usage. Our sample consisted of 98 patients with cognitive impairment, divided into a non-dementia group (N = 34) and a dementia group (N = 64). Most participants in the dementia group had Alzheimer's disease or related conditions. Through multiple logistic regression, the model equation aimed to determine whether a subject's functional disability indicated a potential dementia diagnosis.

Results

The optimal model equation identified the microwave oven and air conditioner as key factors, achieving an area under the curve (AUC) of 0.78. Additionally, analysis by age and gender enhanced the discriminative power of the results.

Conclusions

Our proposed scoring system can efficiently determine the degree of life disability by assessing appliance usage, demonstrating comparable discriminatory ability to existing scales.
