    The branches of the external carotid artery can show variation, and common trunks are occasionally found. Because the common trunks such as the thyrolingual, linguofacial, and thyrolinguofacial trunks are formed from two or all of the superior thyroid artery, lingual artery, and facial arteries, maxillofacial trunks are extremely rare. We report a rare case with the maxillofacial and thyrolingual trunks identified by three-dimensional computed tomography angiography.

    ABSTRACT: The extraoral approach with the risk of facial nerve injury and the visible scar is commonly performed for an elongated styloid process, because intraoral styloidectomy is challenging for the deep and limited surgical field. The authors report minimally invasive intraoral styloidectomy using an endoscope with tissue retractor. A 57-year-old female was referred to our department with a left pharyngeal foreign body sensation and pharyngeal pain after head turning and neck compression. Clinical and radiological diagnosis was an elongated styloid process (Eagle syndrome). Because the styloid process could be palpated intraorally, the patient underwent endoscopically-assisted intraoral styloidectomy without tonsillectomy under general anesthesia as minimally invasive surgery. The styloid process was resected safely with a piezoelectric surgical device under endoscopic guidance. The postoperative course was uneventful without complications.

    ABSTRACT: The purpose of this study was to investigate the clinical anatomy of the feeding artery of the submandibular gland (SMG). A total of 199 SMG removals were performed in extraoral or intraoral approach with/without endoscopic assistance by a single surgeon. The feeding artery of the SMG was always identified intraoperatively and recorded in the operation record. The clinical anatomy of the feeding artery of the SMG was investigated based on the intraoperative findings. The facial artery was ligated and transected for the involvement of tumor or severe inflammation in 16 (8.0%) procedures. The feeding artery was investigated in the remaining 183 procedures. The mean number of the glandular branch was 1.5. There were 1 branch in 98 procedures (53.6%), 2 branches in 80 procedures (43.7%), and 3 branches in 5 procedures (2.7%). Detailed anatomical knowledge of the feeding artery of the SMG is useful for proper dissection of the SMG and preservation of the facial artery.

    PURPOSE: Radioisotope (RI) tracers are generally used for preoperative mapping of sentinel lymph node (SLN) and intraoperative detection with a portable γ probe. However, the use of RI has several limitations. Therefore, a method without RI is required for the widespread application of SLN biopsy. The purpose of this study was to evaluate the feasibility of SLN biopsy with a handheld cordless magnetic probe following magnetic resonance lymphography (MRL) using superparamagnetic iron oxide (SPIO) and for clinically N0 early oral cancer. MATERIALS AND METHODS: MRL using SPIO and SLNB with the handheld cordless magnetic probe were performed for 27 patients with clinically N0 early oral cancer. RESULTS: In all 27 patients (100%), SLNs were detected by MRL, and the total and mean number of SLNs were 73 and 2.7, respectively. All SLNs identified by MRL were detectable using the magnetic probe in all patients. CONCLUSIONS: SLNB with handheld cordless magnetic probe following preoperative SLN mapping by MRL using SPIO is feasible, without RI use, for neck management in cases of clinically N0 early oral cancer.

    ABSTRACT: The purpose of this study was to investigate the anatomical relationship between the lingual nerve and submandibular duct. This study included 1403 patients with submandibular or sublingual gland diseases who underwent intraoral removal of submandibular gland sialoliths, submandibular glands, or sublingual glands. Of all patients, 33 patients underwent bilateral surgeries. All surgeries were performed a single surgeon, and the anatomical relationship between the lingual nerve and submandibular duct was always identified intraoperatively and recorded in the operation recorded. The anatomical relationship was investigated based on the intraoperative findings. The lingual nerve which crosses above the submandibular duct was detected in 8 of 1436 sides (0.6%). There were 4 in the right sides and 4 in the left sides. The lingual nerve below the submandibular gland was seen in 99.4%. Although the lingual nerve crosses above the submandibular duct with a rarer incidence, surgeons should beware of injuring the lingual nerve during intraoral salivary gland surgery.

    In oral and maxillofacial surgery such as orthognathic surgery and open treatment of maxillofacial fracture, plate fixation is commonly performed. Although the removal of titanium miniplates is controversial, the plates are removed routinely or symptomatically. By excessive torque during the screw removal, stripping or breaking of screw heads may occur. Although miniplates and screws are used with angled screwdrivers for osteosynthesis in bilateral sagittal split osteotomy (BSSO) or open treatment of mandibular angle or subcondylar fracture, the removal of a damaged screw, especially with the angled screwdriver, is very difficult. Therefore, we report a simple method with a sterilized surgical glove for removal of the stripped screw.

    Foreign body-induced sialolith is very rare. We report minimally invasive sialendoscopic removal of gold filament thread-induced sialolith in the duct of the parotid gland. A 51-year-old woman with recurrent swelling of the left parotid gland was referred to our hospital. She had undergone insertion of 0.1-mm-diameter gold filament threads into the subdermal skin for facial rejuvenation previously. Computed tomography showed many gold filament threads in the subdermal skin and a sialolith (9.5×4.1×7.9mm) including a gold filament thread in the left parotid duct. The patient underwent endoscopic removal of the sialolith using a 1.6-mm-diameter sialendoscope and Holmium laser under general anesthesia. The sialolith was completely removed with basket and forceps after laser fragmentation, and the broken fragments contained gold filament thread. There was no recurrence of parotid gland swelling after the removal.

    下顎枝矢状分割術の骨接合にはチタン製プレートが使用されることが多いが，術後のプレート関連合併症についての検討は少ない．今回われわれは当科で下顎枝矢状分割術を含む顎矯正手術を施行し，下顎に生じたプレート破折3 例について検討した．プレート破折は術後1 ～ 5 か月の間で生じ，いずれの症例もプレートにはベンディングによるステップが付与されており，近位骨片の切断面に近いスクリューホールでプレート破折が生じていたが，顎間ゴム牽引を継続していたため咬合の偏位は認めなかった．プレート破折のリスクが高いと考えられる症例では，プレート強度の高いデザインや厚みのプレートを選択すべきである．

    症例は31歳男性で、左側顎下部の腫脹を主訴とした。他院にて顎下型ラヌーラの診断で約10年間OK-432の局注療法を受けたが改善しなかった。腫瘤は弾性軟で無痛性であり、CTでは多房性嚢胞状病変が左側顎下腺の後方かつ広頸筋の内側から胸鎖乳突筋の内側まで進展し、MRIではT2強調像で高信号を呈していた。超音波ガイド下穿刺吸引では淡黄色で漿液性の内容液を認め、リンパ管奇形と診断した。全身麻酔下に病変を一塊に摘出し、病理組織学的にもリンパ管奇形と診断された。術後に神経麻痺は認めず軽快退院し、5年以上経過して再発は認めず経過良好である。

    症例は49歳男性で、左側頬部に疼痛を伴う強い腫脹を認めたため、他院耳鼻咽喉科・頭頸部外科を受診した。CTなどの画像検査が行われ、左側耳下腺唾石症および急性化膿性耳下腺炎と診断された後、抗菌薬の投与が行われた。消炎治療後、内視鏡下での低侵襲な治療を希望され当科紹介受診となった。反復する炎症により、初診時点ですでに唾石が導管外に逸脱していた可能性を考慮し、手術はまず唾液腺内視鏡で導管内を観察し、唾石が導管外に逸脱している場合には皮膚自壊部を切開して口外法で唾石摘出する方針とした。術中所見より唾石が導管内の肉芽形成部より導管外に逸脱したものと考え、口外法での唾石摘出術に切りかえた。術後2ヵ月目には皮膚の炎症症状は完全に消失し、左側耳下腺開口部より良好な唾液の流出が認められた。

    48歳女。舌の腫瘤を主訴に近在歯科を受診し、舌癌が疑われ、精査加療目的に当科紹介受診となった。左側舌前方の舌下面に23×22mm大の腫瘤性病変を認め、生検で高分化型扁平上皮癌と診断し、舌部分切除術とSliding tongue flapによる再建術を行った。永久標本の病理組織学的検査で左側顎下のセンチネルリンパ節に転移を認めたため、術後1ヵ月時に左側保存的頸部郭清術を施行したが、郭清組織内のリンパ節に転移は認められなかった。以後外来で経過観察し、術後わずかな舌の短縮を認め、右側と前方の舌運動に軽度の制限を認めたが、舌運動は概ね良好であった。術後3ヵ月時の言語評価では、発語明瞭度は96%で、単音節は術前に比べて3%の低下を認めたが、単語検査では音に歪みはなく、実用的な会話には影響を認めなかった。味覚障害もなく、術後7年7ヵ月の現在まで再発や転移は認めていない。

    67歳男。右側耳下腺腺体内唾石症に対し前医で口外法による唾石摘出術を受けたが、術後に唾液が耳下腺外へ溢出し、右側耳下腺部に貯留嚢胞が生じた。耳下腺浅葉切除を施行されたが唾液の溢出は改善せず、貯留嚢胞は残存した。保存的治療として唾液の穿刺吸引処置が頻回に行われたが再発を繰り返し、当科に紹介された。唾液を穿刺吸引した後にOK-432を貯留嚢胞内に局所注射する治療法を試みたが再発した。保存的治療では治癒困難と判断し、貯留嚢胞の導管狭窄部を口腔内から切除したうえで開窓術を行う方針とした。耳下腺乳頭周囲を円形状に切開し、上下方向に切開を延長し、導管から周囲組織を丁寧に剥離しながら深部へ進むと貯留嚢胞部に到達した。貯留嚢胞の断端の直径が2cm程度になる位置で導管を含めるように切除し、唾液を排出させた。貯留嚢胞の断端を頬粘膜に縫合することで開口部を形成し、手術を終了した。術後経過は良好で、1年2ヵ月後の現在、形成した開口部は縮小しているものの開存しており、唾液が排出されることで貯留嚢胞は生じていない。

    症例は50歳代男性で、右側顎下部の腫脹が食事時に生じたため、近耳鼻咽喉科を受診した。右側顎下腺唾石症と診断され、顎下腺摘出術が必要と説明されたため、唾液腺内視鏡での低侵襲な治療を希望して当科受診となった。唾石は顎下腺腺体上部に存在していたため、内視鏡下での唾石摘出が困難であった場合には、内視鏡をガイドにした口内法で唾石の摘出を行い、それが困難であった場合には顎下腺摘出術が必要になることを患者に説明し、同意を得たうえで全身麻酔での手術を計画した。細い唾液腺内視鏡は腺体内まで挿入可能であり、主導管から2分岐目に唾石を確認できた。唾液腺内視鏡のワーキングチャンネルからバスケット鉗子を挿入し唾石を把持した。唾石が類円形であったこともあり、開口部を切開せずに摘出可能であった。術後2年3ヵ月経過するも顎下腺の腫脹や唾仙痛は認めず、経過良好である。

    症例は10歳男児で、食事時の右側顎下部の腫脹を認めた。CTにて右側顎下腺唾石症と診断した。右側口底後方粘膜に自壊部を認めたが、排膿は認めなかった。嘔吐反射が強く口底部の触診は困難であった。右側舌下小丘部からの唾液の流出は認めなかった。CTでは右側顎下腺移行部に56×56×42mmの硬組織像を認めた。臨床所見から、急性化膿性顎下腺炎により口底粘膜から排膿ととともに唾石が排出された後に、自壊部から顎下腺移行部に生じた肉芽組織が導管を閉塞させていると考え、閉塞性顎下腺炎と診断した。抗菌薬による消炎を行い、唾液腺内視鏡による閉塞物の摘出術を計画した。全身麻酔下で内視鏡手術を行った。外来経過観察時の唾液の流出は良好となり、以後、顎下腺の腫脹は生じていない。

    <p>Sialoliths commonly occur in the submandibular grand, and parotid gland sialoliths are rare. We report the endoscopic removal of a parenchymal sialolith of the parotid gland. A 56-year-old man with right parotid swelling was referred to our department for endoscopic removal of a parenchymal sialolith of the parotid gland. Computed tomography showed that the parenchymal sialolith measured 3.6 × 2.5 × 1.7 mm. Diagnostic sialendoscopy was performed with the patient under local anesthesia, and the parenchymal sialolith was found in the endoscopic view. Therefore, the patient underwent endoscopic removal of the sialolith under general anesthesia as minimally invasive surgery. Complete removal was performed using a microbur and basket, and the postoperative course has been uneventful for 1 year 9 months.</p>

    Warthin tumor is a benign salivary gland tumor which usually occurs in the parotid gland, but rarely in the cervical lymph node. We report a Warthin tumor in a contralateral parotid gland lymph node that mimicked metastasis of oral floor cancer on FDG-positron emission tomography/computed tomography （PET/CT）. For treatment of oral floor cancer, a man in his 70s was referred to our department. CT showed a 14×11×9mm mass below the contralateral parotid gland. FDG-PET/CT showed abnormal accumulation （SUVmax＝4.7） of FDG in the mass. Because differential diagnosis of lymph node metastasis and Warthin tumor was difficult, CT lymphography （CTL） was performed. Although CTL revealed one sentinel lymph node in the right submandibular region, there was no lymphatic flow to the contralateral parotid lymph node. Therefore, the patient underwent tumor resection and biopsy of both lymph nodes. The pathological diagnosis of the contralateral parotid lymph node was Warthin tumor. There was no evidence of recurrence and metastasis 11 months after surgery.

    <p>Fibrous dysplasia (FD) is a bone lesion characterized by replacement of the normal bone structure with fibro-osseous connective tissue. Maxillofacial FD causes facial asymmetry. However, reduction surgery via an intraoral approach is very challenging and can provide insufficient results. We report navigation-guided bone reduction surgery for maxillary FD and obtained favorable results. A 23-year-old man with FD of the maxilla underwent bone reduction surgery using a navigation system after preoperative planning including the mirroring method. The patient could obtain facial symmetry after surgery, and there was no facial asymmetry 9 years 3 months after surgery.</p>

    60代男性。左側耳下腺導管内唾石症と診断され、内視鏡下唾石摘出術を行う予定であった。しかし、初診から3週間後、左側頬部の腫脹や硬結の増大を認め、その中央部は自潰していた。自潰部からは排膿を認めたため、抗菌薬による消炎処置を継続したところ、1週間後に唾石と思われる石灰化物2個が自潰部から自然排出した。エコーでも唾石の残存がなく、1ヵ月経過後には左側頬部の腫脹は消失し、自潰部は完全に閉鎖していた。本例は、唾石に感染を併発したことで膿瘍が形成され、導管外の膿瘍腔へ唾石が逸脱し、皮膚瘻孔からの排膿とともに唾石が自然排出されたと考えられた。

    7歳男児。左顎下部の腫脹を主訴に前医を受診し、両側顎下腺導管移行部唾石と診断された。唾石のサイズが小さいことから、自然排出を期待して経過観察された。10ヵ月後に前医を再診し、唾石の位置と大きさに変化がみられなかったため顎下腺摘出術を提案されたが、本人と家族は内視鏡による低侵襲な治療を希望し、当科に紹介された。左側の唾石は1.7×1.7×1.1mm大で、初診2ヵ月後に開口部付近まで移動し、外来局所麻酔下に容易に摘出することができた。右側は3.0×1.6×1.5mm大で、初診8ヵ月後に開口部付近まで移動し、外来で容易に摘出することができた。

    口腔癌ではリンパ節転移の有無が予後因子となるためその制御は極めて重要であり，早期かつ低侵襲に正確な頸部リンパ節転移の診断を行う必要がある．
    申請者はCT lymphographyとインドシアニングリーン蛍光法による新しいセンチネルリンパ節生検を確立してきたが，ヨードアレルギー，喘息や腎機能障害のある患者には造影剤が投与できない．また，CTを繰り返し撮影するため，放射線被曝の問題もある．
    そこで，磁性造影剤とMRIによる磁気法に着目した．本研究では，早期口腔癌症例に対して磁気法とラジオアイソトープ（RI）法によるセンチネルリンパ節生検の非劣性試験を行うことで，新しい磁気法によるセンチネルリンパ節生検の有効性を検証する. RI法に比べて非劣勢であることを証明できれば，本法は多くの施設で実施可能となり，口腔癌患者のQOLの向上や予後の改善が期待できると考えられる.
    具体的には「頸部リンパ節転移を伴わない早期口腔癌患者に対して，放射線被曝のない新しい革新的なセンチネルリンパ節生検を実現するために，磁性造影剤を用いたMR Lymphography（MRL）によるセンチネルリンパ節の同定法を確立すること」および「磁性造影剤のリンパ節への流入パターンにより，早期転移診断が可能かどうか検討すること」を本研究の目的とした．前者では磁性造影剤を局所投与し，後者では静脈内投与する予定である．とくにMRLおよびMRIは，造影剤と撮影料以外の費用は要せず，当院で稼働しているMRIでの撮像が可能であるため，本研究により頸部のセンチネルリンパ節の描出やリンパ節転移の早期診断が可能となれば，多くの施設でヨード造影剤を使用しない診断法として普及すると考えられる．

    口腔癌において造影超音波検査を用いた頸部リンパ節転移診断法が開発できれば，早期に正確な転移診断を行える可能性があると考えた.そこで造影超音波検査 を用いた口腔癌頸部リンパ節転移の革新的早期診断法を確立することを本研究の目的とした．具体的には，「局所進行症例を含む口腔癌患者において，造影超音 波検査による頸部リンパ節転移の診断精度を造影CT検査による転移診断と比較し，同等の診断精度を得ることができるか」および，「早期口腔癌患者の潜在的リンパ節転移を造影超音波検査により同定することが可能か」を検討することとした. 前者では造影CTが撮影された局所進行症例を含む口腔癌患者40例に対してソナゾイドを注射用水2mlで懸濁し，0.015mL/kgを静脈内投与する.投与と同時に頸部リンパ節の超音波検査を行い，造影されたリンパ節の診断結果を造影CT検査での診断結果と比較し，造影CT検査と同等の転移リンパ節診断が可能かを検証する方針とした.また後者では，早期口腔癌患者20例を対象として，CT lymphographyを撮影し，センチネルリンパ節を同定する.手術直前に，口腔癌周囲4か所に各0.5mlのソナゾイドを局注し，造影超音波でセンチネルリンパ節のマッピングを行う.摘出したリンパ節は病理組織学的診断により転移の有無を確認する.術中の造影超音波画像と比較し，潜在リンパ節転移の有無により，造影効果や血流動態などに特有の所見があるか検討する．造影超音波検査を用いセンチネルリンパ節を同定，転移の有無を評価する方針とした. 今年度は異動があり，異動先の施設の倫理委員会で承認を得たため，超音波機器の整備ができ次第，患者のリクルートを開始する予定である．
