    Although kidney cancer arises from a variety of nephron cells with diverse characteristics and develops intra-tumor heterogeneity, a model to elucidate these complexities is incompletely developed. Here, we report a genetically engineered human iPS cell-derived kidney organoid (HKO) model, which may recapitulate an early stage of renal tumorigenesis. When we overexpressed VCL-ALK fusion gene, a renal oncogene, in HKO, tubular cells proliferated and this proliferation was sustained under long-term culture or hypoxic conditions. In addition, the proliferating tubular cells migrated into the renal parenchyma of host mice upon transplantation. The deconvolution analysis and immunohistochemistry revealed that proliferating tubular cells could be arrested at an immature tubular progenitor stage with increased expressions of LHX1 and JAG1, critical regulators of nephrogenesis. Our HKO model advances our understanding of renal tumorigenesis in the context of the trajectory of nephron development. Implications: Dysregulated nephron developmental machinery leads to aberrant cell proliferations of immature renal tubules.

    There is a great unmet medical need for development of molecularly characterizing modalities to assist in the complex classification of rare kidney cancers, some of which are diagnosed as unclassified renal cell carcinoma (unclassified RCC) due to complex histology. Here we show utility of the comparative transcriptome atlas as an assistive modality for complex classification of rare kidney cancers. Whereas whole genome sequencing (WGS) of 52 rare kidney cancers identifies very few clinically significant variants in a subset of cases, unsupervised clustering results of RNA-seq data from 219 renal tumors including 140 rare kidney cancers are largely consistent with the histological classification based on WHO2022 classification. Additionally, the comparative transcriptome atlas may provide an opportunity to define the molecular characteristics of unclassified RCC and might predict patient outcome. These findings support the comparative transcriptome atlas as an assistive modality for complex classification of rare kidney cancers.

    INTRODUCTION: There are no previous reports of solitary renal metastases from urothelial carcinoma with trophoblastic differentiation, a rare bladder cancer subtype that is pathologically hCGβ positive. CASE PRESENTATION: A 77-year-old male with urothelial carcinoma with trophoblastic differentiation underwent robot-assisted radical cystectomy following neoadjuvant chemotherapy. Pathological examination revealed urothelial carcinoma, classified as ypT2b and ypN0 with detection of focal hCGβ positivity. Postoperatively, serum hCGβ levels decreased from 1.6 to < 0.2 mIU/mL. At the 9-month follow-up, serum hCGβ was elevated to 20.1 mIU/mL with no recurrence on PET-CT. Gemcitabine-cisplatin chemotherapy was initiated; however, a solitary renal tumor was detected. Partial nephrectomy confirmed that the tumor was a renal metastasis of bladder cancer. Serum hCGβ levels decreased and remained < 0.2 mIU/mL, even 20 months after partial nephrectomy. CONCLUSION: We report a case of urothelial carcinoma with trophoblastic differentiation and elevated serum hCGβ levels, in which a solitary renal metastasis was successfully resected by robot-assisted partial nephrectomy.

    Biphenotypic sinonasal sarcoma is a newly established tumor entity that is associated with distinct clinicopathological findings. Biphenotypic sinonasal sarcoma is a rare, low-grade spindle cell sarcoma that arises in middle-aged females, exclusively in the sinonasal tract. A fusion gene involving PAX3 is detected in most biphenotypic sinonasal sarcomas, which aids in its diagnosis. Here, we report a case of biphenotypic sinonasal sarcoma with its cytological findings. The patient was a 73-year-old woman who presented with purulent nasal discharge and dull pain in the left cheek area. Computed tomography showed a mass extending from the left nasal cavity to the left ethmoid sinus, the left frontal sinus, and the frontal skull base. She underwent a combined transcranial and endoscopic approach for en bloc resection with a safety margin. Histologically, spindle-shaped tumor cells have been thought to proliferate mainly in the subepithelial stroma. Here, nasal mucosal epithelial hyperplasia was noted, and the tumor had invaded the bone tissue accompanying the epithelial cells. Fluorescence in situ hybridization (FISH) analysis showed a PAX3 rearrangement, and next-generation sequencing identified a PAX3::MAML3 fusion. Based on FISH, split signals were observed not in respiratory cells but in stromal cells. This indicated that respiratory cells were non-neoplastic. In the diagnosis of biphenotypic sinonasal sarcoma, the inverted growth of the respiratory epithelium can be a diagnostic pitfall. FISH analysis using a PAX3 break-apart probe is helpful not only for an accurate diagnosis but also for detecting the true neoplastic cells.

    AIM: Immune-related adverse events (irAEs) caused by immune checkpoint inhibitors are reported in all organs; however, the frequency of liver injury is low compared to irAEs in other organs. We describe a case of fulminant hepatitis after administration of the first dose of nivolumab for the management of esophageal cancer. METHODS: A man in his 80s was treated with nivolumab as a second-line therapy after his overall health worsened during preoperative chemotherapy for esophageal cancer. He was admitted to the hospital as an emergency case 30 days later with complaints of vomiting, following which acute liver failure was diagnosed. RESULTS: The patient developed hepatic encephalopathy on the third day after admission and died on the seventh day. The pathological results showed sub-extensive spread hepatocellular necrosis throughout the liver, and immunostaining confirmed the presence of CD8-positive cells, which is consistent with irAEs. CONCLUSIONS: Immune checkpoint inhibitors have proven to be effective for the treatment of malignant tumors, and although fatalities due to acute liver failure are extremely rare, such cases have been reported previously. Among the immune checkpoint inhibitors, anti-programmed death-1 receptor is associated with less hepatotoxicity. However, even a single dose of this treatment can cause acute liver failure, which could be fatal.

    BACKGROUND: Rhabdomyosarcoma (RMS) is a rare malignant mesenchymal tumor, primarily affecting children. Occurrence in the uterine corpus of adults, particularly postmenopausal women, is extremely uncommon. CASE PRESENTATION: A 60-year-old postmenopausal nulligravid woman presented with abnormal genital bleeding for three months. Pelvic examination revealed a friable mass extending from the uterine corpus. Biopsy confirmed embryonal RMS. She underwent modified radical hysterectomy with bilateral salpingo-oophorectomy for complete surgical resection. One cycle of adjuvant VAC chemotherapy (vincristine, actinomycin D, and cyclophosphamide) was administered but discontinued at her request. No residual disease was observed postoperatively, and she remains disease-free 12 months after surgery. CONCLUSION: Adult-onset uterine corpus embryonal RMS is rare, and standardized treatment protocols are lacking. Early diagnosis, complete surgical resection, and individualized therapy are essential. A literature review highlights clinical characteristics, treatment strategies, and prognostic considerations.

    INTRODUCTION: The diagnosis of systemic transthyretin amyloidosis based on prostate biopsy is very rare, and we report this case with a literature review. CASE PRESENTATION: A 73-year-old male patient was referred to our department due to an increased serum prostate-specific antigen level at 10.4 ng/mL while receiving benign prostatic hypertrophy treatment. Prostate needle biopsy was performed, and the pathology revealed transthyretin amyloid deposits in the prostate tissue without adenocarcinoma. Subsequent 99mTc pyrophosphate scintigraphy confirmed the accumulation in the myocardium. The patient was diagnosed with cardiac transthyretin amyloidosis. CONCLUSION: Amyloidosis is a systemic disease where amyloid is deposited in multiple organs, causing various dysfunctions. Considering both our case and previous reports, prostatic amyloidosis may frequently be associated with systemic involvement. The treatments including oligonucleotide therapeutic agents and transthyretin-stabilizing drugs will be more effective with early diagnosis and early treatment, and various symptoms suspicious of systemic amyloidosis should not be missed.
