    OBJECTIVES: The etiology of type A acute aortic dissection (TAAAD) remains unclear. This study aimed to investigate whether the diameter of the common femoral artery (CFA) could serve as a predictor of TAAAD. METHODS: This retrospective study included 100 patients aged 20-70 years who underwent surgery for TAAAD and had postoperative contrast-enhanced (CT) performed before discharge (TAAAD group). The control group comprised 100 patients who underwent elective surgery for stage ≤ 2 colorectal cancer without preoperative bowel obstruction or perforation between April 2021 and March 2024. CFA diameters measured on postoperative CT images in the TAAAD group were compared with those measured on preoperative CT images in the control group. RESULTS: The TAAAD group had a higher proportion of men (76% vs. 59%; p = 0.016), larger body surface area (1.78 [1.65-1.91] vs. 1.70 [1.57-1.83]; p = 0.004), younger age (57 [52-64] vs. 60 [54-67]; p = 0.011), and larger CFA diameter (11.2 mm [10.1-12.1] vs. 9.0 mm [8.5-9.8]; p < 0.001). Receiver operating characteristic curve analysis for CFA diameter demonstrated good discriminatory performance (area under the curve, 0.88; 95% confidence interval, 0.84-0.93). An optimal cutoff value of 10.5 mm yielded a sensitivity of 70% and a specificity of 90%. In multivariable logistic regression analysis, CFA diameter remained significantly associated with TAAAD (OR 5.45, 95% confidence interval 3.47- 9.31, p < 0.001). CONCLUSIONS: The CFA diameter was significantly larger among patients with TAAAD. These findings suggest that CFA diameter may be associated with the risk of TAAAD.

    BACKGROUND: Postoperative atrial fibrillation (POAF) after cardiac surgery requires prompt intervention. We compared landiolol with verapamil for POAF treatment. METHODS AND RESULTS: This randomized trial enrolled 179 patients; 45 developed POAF. Landiolol achieved higher sinus rhythm conversion at 8 h (73.3% vs 16.7%; P=0.0016), but not at 12 h. No differences were observed in recurrence, adverse events, or intensive care unit stay. CONCLUSIONS: Landiolol facilitates earlier rhythm conversion without clear short-term clinical benefit.

    OBJECTIVE: In surgery for type A acute aortic dissection, we perform supracoronary reconstruction with fibrin glue when the sinus of Valsalva has no intimal tears, and the diameter is ≤ 45 mm. This study aimed to clarify the surgical outcomes with fibrin glue, the incidence of aortic root events, and the influence of the preoperative severity of aortic regurgitation (AR) on aortic root events. METHODS: Among 621 patients who underwent central repair in the past 20 years, we included 546 who underwent supracoronary reconstruction (87.9%). We closed the false lumen using fibrin glue and placed inner and outer polytetrafluoroethylene felt strips. The mild AR group included patients with no, trivial, or mild preoperative AR, whereas the severe AR group included patients with moderate or severe AR. RESULTS: Acute aortic root events occurred in 19 patients (3.5%), and the incidence was higher in the severe AR group than in the mild AR group (9 [5.9%] vs 8 [2.2%]; P = .029). The long-term outcomes were investigated in 486 patients who underwent supracoronary reconstruction and were subsequently discharged. Late events occurred in 13 patients (2.7%), the 5-year and 10-year event-free rates were 98.4% and 95.1%, respectively. The pseudoaneurysm-free rate was 99.8% at 10 years. The 5-year and 10-year event-free rates were lower in the severe AR group than in the mild AR group (97.1% vs 99.3% and 85.8% vs 98.5%, respectively; P = .001). CONCLUSIONS: Supracoronary reconstruction can be performed safely with fibrin glue with excellent long-term outcomes. Preoperative AR is a risk factor for aortic root events.

    OBJECTIVE: We reviewed the treatment outcomes for aortic arch aneurysms in elderly patients aged > 80 years, and discussed the risk factors for each technique. METHODS: Octogenarians who underwent aortic arch aneurysm repair between 2007 and 2021 were included. Fifty-four patients (23 in the total arch replacement [TAR] group and 31 in the thoracic endovascular aortic repair [TEVAR] group) were included in the study. The early- and mid-term outcomes and risk factors for all-cause mortality were examined in each group. To examine timely surgical outcomes, cases of true aneurysms were included, whereas dissected aneurysms and emergency cases due to rupture or other causes were excluded. RESULTS: No significant differences in 30-day mortality (0% in the TAR group and 5.4% in the TEVAR group) and in-hospital mortality (7.7% in the TAR group and 8.1% in the TEVAR group) were observed between the two groups. The survival rates at 5 years were 82% and 65% in the TAR and TEVAR groups, respectively, without significant difference. The aorta-related averted mortality was 91% and 81% in the TAR and TEVAR groups, respectively, without significant difference. No significant difference in the freedom from aortic events was also observed between the two groups. Previous ischemic heart disease was a significant risk factor for all-cause mortality in the TAR group. No significant risk factors were identified in this group. CONCLUSION: The choice of procedure was reasonable when considering frailty. Endovascular repair may be a good treatment option for patients with a history of ischemic heart disease.

    PURPOSE: Retrograde type A aortic dissection (RTAD) represents a serious complication of endovascular treatment for type B aortic dissection (TBAD). To avoid RTAD, it is recommended to land the proximal end of the stent graft in a non-dissected aortic segment. In this study, we investigated whether landing in the dissection area increased the number of events at the proximal site. METHODS: We conducted a retrospective review of endovascular treatments for TBAD at a single institution between 2009 and 2022. Patients were divided into two groups: group A, with a proximal landing zone entirely within the dissected area, and group B, with the proximal extent of the seal zone in the non-dissected area. We evaluated the occurrence of proximal events, including RTAD, and examined long-term outcomes to assess the validity of landing in the dissection area. RESULTS: The study included eighty-nine patients who underwent endovascular treatment for TBAD. New intimal tears in the proximal landing site occurred in 3 cases (3.4%), with 1 case (2%) in group A and 2 cases (5.1%) in group B, showing no significant difference. Among the three cases, one (1.1%) in group B with zone 2 landing resulted in RTAD. At 60 months, the overall survival was 85%, and freedom from aorta-related mortality was 88%, with no significant difference between the groups. CONCLUSION: Even if the proximal landing is in a dissected area, a treatment strategy performed in zone 3 without proximal landing in zone 2, seeking a non-dissected area, can still provide sufficient therapeutic effects.     症例は70歳，男性．主訴は胸背部痛，腹痛，両下肢痛．来院後の臨床症状経過・造影CT所見から腸管・下肢・脊髄虚血を伴う急性A型大動脈解離と診断した．上行大動脈の偽腔は血栓閉塞しており，entryは近位下行大動脈に認め，明らかなre-entryは認めなかった．胸部下行大動脈から腹部大動脈にかけて真腔は高度に狭小化，腹腔動脈，上腸間膜動脈は狭小化した真腔から分枝，多数の肋間動脈は部分血栓化した偽腔から分枝していた．人工血管置換術によるentry閉鎖では偽腔灌流が低下し肋間動脈の血流が低下してしまうこと，腸管虚血評価も必要であったため，開腹での腹部大動脈開窓術を行った．術後造影CTで真腔拡大と偽腔の良好な血流を認め，臨床症状も改善を認めた．術後経過は良好で術後30日に退院した．急性A型大動脈解離に対してはentry切除を行う人工血管置換術が第一選択だが，上行大動脈が保存加療可能であり多臓器のmalperfusionを伴う場合，外科的開窓術は有用な治療選択肢であると考えられた．

    Hypoxia during one-lung ventilation is a significant problem in descending aortic surgery via left thoracotomy.     A 59-year-old man with no prior neurological deficits developed a massive stroke during the repair of a double-barreled acute type A aortic dissection with major entry in the ascending aorta and an occluded brachiocephalic artery. As right cerebral ischemia was alleviated by the circle of Willis, the patient was alert and conscious preoperatively. Nevertheless, the thrombus in the right carotid artery induced a severe postoperative right cerebral embolism. In conclusion, occlusion of the carotid artery is a risk factor of postoperative severe stroke, even in patients without neurological symptoms preoperatively.

    OBJECTIVE: Excessive prostaglandin E2 production is a hallmark of abdominal aortic aneurysm (AAA). Enhanced expression of prostaglandin E2 receptor EP4 (prostaglandin E receptor 4) in vascular smooth muscle cells (VSMCs) has been demonstrated in human AAAs. Although moderate expression of EP4 contributes to vascular homeostasis, the roles of excessive EP4 in vascular pathology remain uncertain. We aimed to investigate whether EP4 overexpression in VSMCs exacerbates AAAs. Approach and Results: We constructed mice with EP4 overexpressed selectively in VSMCs under an SM22α promoter (EP4-Tg). Most EP4-Tg mice died within 2 weeks of Ang II (angiotensin II) infusion due to AAA, while nontransgenic mice given Ang II displayed no overt phenotype. EP4-Tg developed much larger AAAs than nontransgenic mice after periaortic CaCl2 application. In contrast, EP4fl/+;SM22-Cre;ApoE-/- and EP4fl/+;SM22-Cre mice, which are EP4 heterozygous knockout in VSMCs, rarely exhibited AAA after Ang II or CaCl2 treatment, respectively. In Ang II-infused EP4-Tg aorta, Ly6Chi inflammatory monocyte/macrophage infiltration and MMP-9 (matrix metalloprotease-9) activation were enhanced. An unbiased analysis revealed that EP4 stimulation positively regulated the genes binding cytokine receptors in VSMCs, in which IL (interleukin)-6 was the most strongly upregulated. In VSMCs of EP4-Tg and human AAAs, EP4 stimulation caused marked IL-6 production via TAK1 (transforming growth factor-β-activated kinase 1), NF-κB (nuclear factor-kappa B), JNK (c-Jun N-terminal kinase), and p38. Inhibition of IL-6 prevented Ang II-induced AAA formation in EP4-Tg. In addition, EP4 stimulation decreased elastin/collagen cross-linking protein LOX (lysyl oxidase) in both human and mouse VSMCs. CONCLUSIONS: Dysregulated EP4 overexpression in VSMCs promotes inflammatory monocyte/macrophage infiltration and attenuates elastin/collagen fiber formation, leading to AAA exacerbation.

    今回われわれは，極めて稀な僧帽弁弁尖の外傷性裂傷による僧帽弁閉鎖不全の1手術例を報告する．症例は42歳の男性．3年前に3階から転落，顔面骨折・骨盤骨折・脳挫傷・肺挫傷および外傷性大動脈解離を発症し，当院に入院した．受傷直後の僧帽弁の逆流は軽度であり，循環動態の異常は認められなかった．入院加療を行い，受傷後18日目に全身状態は改善し退院した．経過観察中，受傷2年後に労作時の息切れが発現し，経胸壁心臓超音波検査にて僧帽弁閉鎖不全の増悪を認めた．受傷3年後，僧帽弁形成術を施行した．術中所見では僧帽弁の前交連側前尖（A1）に裂傷を認め，これによる重症僧帽弁閉鎖不全と考えられた．裂傷部の弁尖縫合により逆流は消失し，術後労作時の息切れも消失した．

    今回われわれは，術前心肺停止を発症した冠動脈瘤破裂の1手術例を経験したので報告する．症例は47歳，女性．タクシー乗車中に意識消失し，救急搬送となった．救急車内で意識回復，当院救命センターを受診したが診察時に心肺停止となった．心臓超音波検査にて心タンポナーデを認め直ちに心嚢ドレナージを施行，自己心拍再開を得た．脳低温療法を施行後精査にて，右冠動脈領域に最大径5mmおよび左冠動脈領域に最大径8mmの冠動脈瘤を認め，両側冠動脈肺動脈瘻を形成していた．心肺停止の原因は冠動脈瘤の破裂と判断，人工心肺下に瘤切除，冠動脈肺動脈瘻閉鎖術を施行した．術中小さな冠動脈瘤の同定にICG navigation system （photodynamic Eye<SUP>&reg;</SUP>）が有用であった．術後12日目に軽快退院した．

    症例は81歳，男性．弓部大動脈瘤の手術予定であったが急性心不全で入院となった．経過中に連続性雑音を聴取するようになり，胸部造影CT検査にて弓部大動脈瘤の左肺動脈穿破と診断され当院転院となった．転院3日後に弓部置換および穿孔部閉鎖術，CABGを施行したが，人工心肺開始時，大動脈肺動脈瘻により循環の維持が困難であった．術後PCPSによる循環補助を要したが2日目に離脱し49日目に退院した．弓部大動脈瘤に伴う大動脈肺動脈瘻は稀な疾患であるが，破裂死のみならずシャント血流過多により致命的な右心不全をもきたし，救命のためには早期の手術が必要である．

    症例は74歳,女性,主訴は労作時息切れ.心臓超音波検査では左室壁運動は良好であり(LVELF70.2%),左房左室の拡大を認めた(LAD53.4mm,LVDd58.5mm).僧帽弁は後尖の肥厚,可動性の低下を認め,弁輪にエコー輝度の増強する部分を認めた.カラードップラーでは,左房後尖側に向かう逆流jetを認めたが,前尖の逸脱は認めなかった.高度僧帽弁閉鎖不全症の診断で手術施行した.僧帽弁を観察すると限局した後尖弁輪の石灰化によって後尖P2が短縮しており,この部位が逆流の原因と考えられた.弁尖矩型切除,弁輪縫縮による僧帽弁形成術を施行した.術後経過は良好であり,術後の心臓超音波検査で僧帽弁閉鎖不全はtrivialまで改善していた.弁輪石灰化を伴う僧帽弁閉鎖不全症に対しては,形成術は不向きとされていたが,本症例のように石灰化が限局している症例に対しては弁尖切除,弁輪縫縮によって形成術は可能であると考えられた.
