    AIM: To explore the role of artificial intelligence (AI) in capturing tacit midwifery knowledge and its potential to enhance nursing and midwifery practices and policies, and examine how AI tools, such as machine learning (ML) and natural language processing (NLP), can facilitate the integration of midwives' experiential knowledge into evidence-based practice and policy. BACKGROUND: Midwifery is largely driven by tacit knowledge gained through personal experience. This knowledge, which is crucial to maternal and newborn care, is often underrecognized in clinical policies. AI presents an opportunity to formalize this knowledge, improve healthcare outcomes, and increase the visibility of midwifery within policy frameworks. SOURCES OF EVIDENCE: A review of recent peer-reviewed literature focusing on studies related to AI in healthcare, midwifery practices, and AI ethics was conducted. Thirty-one sources spanning 2015-2025 were reviewed, emphasizing AI applications in clinical settings and midwifery education. DISCUSSION: AI has the potential to unlock tacit midwifery knowledge, particularly in perineal trauma prevention and clinical decision-making. AI models can provide real-time risk assessments and reduce clinical variability by analyzing qualitative data from experienced practitioners. However, ethical concerns, such as data privacy and algorithmic bias, must be addressed in AI integration. CONCLUSION: AI offers substantial potential for improving midwifery practices by formalizing tacit knowledge and enhancing decision-making. However, successful integration requires careful attention to the ethical, governance, and regulatory issues. IMPLICATIONS FOR NURSING PRACTICE AND POLICY: Policy frameworks should encourage the development of AI tools tailored to midwifery, ensuring that midwives are equipped with the necessary training to work with these systems.

    Background Non-communicable diseases account for 71% of global deaths. Health behaviors are fundamental for prevention. Previous studies have indicated that health literacy, such as functional, interactive, and critical, affects people's health behaviors. Health information is widely available online. Young people tend to access the Internet for information, which highlights the importance of eHealth literacy. Although previous studies have assessed eHealth literacy in single-country contexts, cross-cultural comparisons between high- and middle-income nations remain limited, particularly in understanding its role during global health crises. The COVID-19 pandemic has dramatically accelerated the shift toward seeking digital health information, fundamentally changing how students access and utilize health resources. However, whether eHealth literacy could mitigate the impact of COVID-19 on lifestyle remains unclear. Therefore, this study aimed to examine the impact of eHealth literacy on health behaviors among university students in Japan and Indonesia providing unique insights into cross-cultural differences. Methodology This cross-sectional study used a self-administered, web-based questionnaire. The participants were first- to fourth-year nursing students from universities in both countries. eHealth literacy and health behaviors were evaluated via the e-Health Literacy Scale (eHLS) and Healthy Lifestyle Scale for University Students (HLSUS), respectively. Structural equation modeling was conducted to evaluate the impact of eHLS on HLSUS, adjusting for potential confounders. A multigroup structural equation model was used to assess configural, metric, and scalar invariance. Results In total, 99 and 407 responses were obtained from Japan and Indonesia, respectively (response rates: Japan, 23.9%; Indonesia, 89.6%). The direct effect of eHLS on HLSUS was 0.33; the indirect effect via the impact of COVID-19 was 0.06. The metric invariance, although not scalar, was supported in the model that included data from both countries. Conclusions eHealth literacy universally could influence health behaviors among Japanese and Indonesian university students. However, it did not fully mitigate the effects of COVID-19. Enhancing eHealth literacy could support better health behaviors among university students.

    AIM: Evidence-based indicators are needed to increase the frequency of practicing care for hiesho, which can be a risk factor during childbirth. Such practice will lead to hiesho prevention. This study aimed to develop a Hiesho Care Practice Scale (HCPS) for pregnant women and examine its reliability and validity. METHODS: To develop a HCPS and examine its reliability and validity, we conducted a questionnaire-based survey among nurses who provide hiesho care for pregnant women in Japan. RESULTS: The analysis included 100 participants (recovery rate: 96%). Exploratory factor analysis produced four factors and 20 items. The model's fit indices obtained from the confirmatory factor analysis include goodness of fit of .80, adjusted goodness of fit of .75, comparative fit index of .88, and root mean squared error of approximation of .08. The correlation coefficients for the patient coaching skill evaluation scale for criterion-related validity test ranged from .23 to .51 (p < .01). Cronbach's α was .88, and intraclass correlation coefficient (ICC) was .81. CONCLUSIONS: The HCPS developed in this study consisted of four factors and 20 items, and its reliability and validity were verified. This scale enables the subjective and objective evaluation of hiesho care practice in the nursing profession. It may contribute to standardizing and developing improvement measures for hiesho care among pregnant women by nurses and midwives.

    AIM: To evaluate the effectiveness of an individualized nutritional education program in promoting adequate nutrient intake in pregnant women. METHODS: A stratified randomized controlled trial was conducted. Participants were stratified by factors affecting the primary outcome and randomly assigned to the intervention or control groups. Intervention group participants received an individualized 30-min booklet-based education program in their 2nd and 3rd trimesters; the control group received usual care. The primary outcome was protein intake after the intervention, which was compared between the intervention and control groups. Secondary outcomes included comparing the amount of increase of protein before and after the intervention. Nutrient intake was measured using a self-administered short dietary history questionnaire, and analyses of covariance and t tests were performed. RESULTS: Of the 130 participants, 66 were assigned to the intervention group and 64 to the control group. There was no difference in protein intake between the two groups after the intervention (p = .051, 95% CI [-0.021, 12.4]). Comparing the increase in protein intake before and after intervention, the intervention group was 7.4 g/day higher than that of the control group (p = .040; F = 4.31; effect size = 0.36). CONCLUSIONS: The primary outcome, a comparison of protein intake between the groups after the program, revealed no significant differences. However, on comparing the amount of protein increase before and after the intervention, the intervention group's increase was significantly higher than that of the control group. Results indicate the potential for individualized face-to-face interventions for pregnant women in Japan.

    AIM: Hiesho is a condition characterized by having a cold sensation in the hands or feet (i.e., sensitivity of the hands or feet to cold). This condition is common among Japanese women and is an important health problem. In this study, we aimed to evaluate a hiesho intervention program for nurses called "Preventing Hiesho: Intervention Program for Educating Nurses" in terms of its effectiveness in improving the knowledge and perceptions of nurses about the importance of hiesho care. METHODS: A total of 130 nurses from Japanese hospitals and clinics providing prenatal care participated in our randomized controlled trial. Sixty-eight nurses participated in the "Preventing Hiesho: Intervention Program for Educating Nurses" for 2 weeks (intervention group). Sixty-two nurses received a pamphlet on hiesho care (control group). The responses of the participants to the questionnaire survey were collected by postal mail. An intention-to-treat analysis of the results was conducted. RESULTS: One week after the implementation of the web-based learning program, the scores of knowledge and perceptions of the importance of administering hiesho care were significantly higher in the intervention group than in the control group. CONCLUSIONS: A 2-week web-based learning program for educating nurses on preventing hiesho significantly improved their knowledge and perceptions of the importance of hiesho care.

    フィリピンと日本の看護教育を比較するため、フィリピン大学マニラ校(UPM)と当大学(YCU)のカリキュラムを精査した。日本では看護師になるためには97単位以上の履修が必要であるが、フィリピンでは202単位以上が求められた。フィリピンでは、人口あたりの医師や看護師の数が日本の1/4〜1/5なので、卒業後すぐに活動できる看護師が必要とされた。フィリピンの看護教育は医療の要求するレベルに従って構築されていた。日本では高齢化社会になり、カリキュラムは人生の発達段階に合わせて分類されており、新人看護師は自身の看護技術と現場が期待するレベルのギャップを感じていた。フィリピンではいろいろな環境に適応できるような看護教育を設定しており、日本の看護教育も臨床実習を増やす必要があると考えられた。

    BACKGROUND: In Japan, 85% of pregnant women do not practice antenatal perineal massage. Therefore, we developed a smartphone website to support the practice of antenatal perineal massage. The purpose of the present study was to evaluate the feasibility of our smartphone website. METHODS: Pregnant women were recruited at five hospitals or clinics in Tokyo, Japan. Participants assigned to the smartphone website group (n = 74) were asked to register on the smartphone website. After completing registration, they could login and use all the contents of the website. After giving birth, participants completed a 5-item questionnaire evaluating the acceptability of the smartphone website. Participants assigned to the leaflet group (n = 71) received a leaflet on antenatal perineal massage and completed a similar 4-item questionnaire evaluating the leaflet. Data were collected from April 2014 to November 2014. Data analysis was performed using chi-square and t-tests to analyze responses to close-ended questions, and content analysis was conducted to analyze responses of open-ended questions. RESULTS: In the smartphone website group, 9 women (12.2%) did not register on the smartphone website. Approximately 80% of the women who responded indicated that the smartphone site was easy to understand and useful for practicing antenatal perineal massage. In the smartphone website group, the reply rate for reporting the frequency of massage was 43.6%. Although the ratings and frequency at which the material was accessed tended to be higher in the smartphone website group than in the leaflet group, there were no significant differences. CONCLUSIONS: Most pregnant women in the smartphone website group provided a favorable evaluation for the smartphone website. However, some participants had suggestions for improvement, which need to be incorporated in a revised version of the website. Therefore, the present study's results demonstrate the feasibility of a smartphone website to support the practice of antenatal perineal massage, and they may aid in the development of similar web-based educational material for pregnant women. TRIAL REGISTRATION: This trial was registered with the UMIN Clinical Trials Registry ( UMIN000013979 ) on May 16, 2014.

    BACKGROUND: In Japan, the rate of pregnant women who practice antenatal perineal massage was only 15.1%. AIM: The aim of this study was to develop and evaluate a smartphone website and a leaflet to support antenatal perineal massage practice for primiparous women. METHODS: In a randomised control trial, 161 primiparous women were randomly assigned to a smartphone website group (n=81) or a leaflet group (n=80). Data analysis were by per protocol analysis and intention to treat analysis. FINDINGS: Of the 161 women participants, 47 in the smartphone website group and 49 in the leaflet group completed all questionnaires. Primary outcome was continuance rate (three times a week over a three week period) of antenatal perineal massage practice. The rates by a per protocol analysis were 51.1% in the smartphone website group and 51.0% in the leaflet group, respectively. There was no significant difference between the groups. Moreover, the rates by an intention to treat analysis were 29.6% in the smartphone website group and 31.3% in the leaflet group, respectively. There was also no significant difference between the groups. There were no significant differences in the evaluation of perineal massage, childbirth self-efficacy, satisfaction with efforts towards childbirth, and perineal outcomes following childbirth which were measured as secondary outcomes between the groups. CONCLUSION: There was no significant difference in continuance rate of antenatal perineal massage practice between those using a smartphone website and those with a leaflet, however, the rate was better than no instructions.

    <b>Purpose</b><br>To determine women's perception of antenatal perineal massage and to identify factors impeding their massage practice.<br><b>Methods</b><br>A questionnaire survey was conducted with a purposive sample of 390 women from eight Japanese urban hospitals, clinics and birth centers and who had a vaginal and full-term delivery. The 334 (85.6%) valid responses were analyzed statistically.<br><b>Results</b><br>1. There were 114 women (52.1%) who practiced antenatal perineal massage and 105 women (47.9%) who did not. Among the 114 women who practiced perineal massage, 59.6% continued the massage until childbirth. Of the 105 women who did not practiced the massage, 45.7% felt a resistance to touching their perineum.<br>2. The rate of women taught how to practice antenatal perineal massage by a midwife, and who practiced perineal massage was significantly larger than who had not practiced (<i>p</i>=.000). Conversely, the rate of women who were taught how the massage during their maternity class, and did not practice was significantly larger than who practiced (<i>p</i>=.000).<br>3. Over half of women who knew how to perform antenatal perineal massage felt it was difficult for example: 'how to move fingers','how much pressure to apply', 'how deeply should a finger be inserted into the vagina' and 'how long does massage practice last'. Women who stopped practicing antenatal perineal massage had felt significantly more difficulty in practicing the massage than those who continued until childbirth (<i>p</i>=.012)<br>4. Women who continued to practice perineal massage felt significantly more positive 'effects on preparation for childbirth' and 'effects on childbirth' than who stopped practicing the massage (<i>p</i>=.000).<br>5. Self-efficacy, 'childbirth in my own way', of primiparous who practiced perineal massage was significantly larger than who did not practice (<i>p</i>=.014).<br><b>Conclusion</b><br>Factors impeding antenatal perineal massage practice were: (1) resistance to touching their perineum, (2) lack of knowledge and (3) some difficulty practicing the massage. Moreover, it was important to not only practice perineal massage but also continue in order to redeem the effects. Therefore, midwives and maternity educators need to support pregnant woman in their efforts to continue antenatal perineal massage until childbirth.

    Purpose To determine the degree of mother's perineal pain following childbirth, which occurs in hospitals and birth centers and to identify the nursing care provided for such pain. Method A questionnaire survey was conducted with a purposive sample of 503 women who had a vaginal and full-term delivery from nine Japanese urban hospitals (n=413) and birth centers (n=90). The 425 (84.5%) valid responses were analyzed statistically. Results 1. Factor analysis results indicated that postnatal women experienced: "difficulty sitting", "loss of volition", "difficulties with excretion and cleanliness" and "difficulty in moving". 2. Only a minority of women practiced perineal massage during pregnancy (15.1%) or received perineal care during the postpartum period (14.1%). Women's satisfaction with perineal pain care was significantly lower than with general postnatal care (p<0.001). 3. At least 90% of women learned about perineal pain from midwives or their families, and the majority of advisers about pain were midwives or nurses. Conclusion Postnatal women experienced: "difficulty sitting", "loss of volition", "difficulty moving" and "difficulties with excretion and cleanliness". It is very important to guide pregnant women in perineal massage and develop nursing care for perineal pain management because few women practiced antenatal perineal massage or received perineal pain care during the postpartum period to alleviate the pain.

    本研究は、冷え症改善支援実施率の低い看護職に対し、妊婦を冷え症改善に導くための共通ケアプログラムを作成することである。主な研究実績として、以下の2つがあり、専門の学術誌に投稿し原著として採択された。
    ①妊婦健診に携わる看護職の冷え症ケア実施の実態と影響要因:目的は冷え症ケアの実態を明らかにし、冷え症ケア実施に影響を与える要因について分析することである。結果として周産期において、妊婦健診時の冷え症ケア実施の割合は44.1%と低く、施設別では、助産所(96.9%)に比べて病院(38.6%)と診療所(33.7%)での冷え症ケア実施割合は有意に低かった(p<0.001)。また、冷え症ケア実施に与える大きな影響要因は、「冷え症の学習経験」であることから、今後は妊婦健診に関わる看護職に対し冷え症に対する学習の機会を提供していく必要があることが課題として明確になった。
    ②看護職の冷え症妊婦に対する関わり方の基本姿勢、構成要因の探索抄録:目的は看護職の冷え症妊婦に対する関わり方の基本姿勢の構成要因を探索することである。733名を対象に分析した結果、基本姿勢の構成要因は、「触れて冷えを確認する」「共に冷え症改善行動を考える」が冷え症ケアに特化した因子であり、「共感的態度で傾聴する」「ペースや意向を尊重する」が看護全般に共通する因子として示唆された。次のステップは、本研究で明らかになった基本姿勢を基盤として行動レベルでの具体的なケア尺度の作成することである。

    本研究の目的は出産後早期の女性を対象とした身体回復を促進する温罨法ケアプログラムを開発し、その効果を検証することである。2021年度は2020年度にCOVID-19の影響で開催延期となった国際学会にて、身体回復につながる会陰裂傷予防に関する研究について、発表を行った。また、会陰裂傷予防に関する臨床判断について、助産所助産師2名を対象にインタビューを行い、妊娠中からの冷えの状況をアセスメントすることや分娩期に温めるケアを実践していることが明らかとなった。本調査により、出産後早期の身体回復の予防的視点から、温罨法ケアは産褥期だけでなく、妊娠期や分娩期においても重要なケアであることが示唆された。

    Support from nursing professionals is important to improve pregnant women’s hiesho through self-care. As such, this study first clarified the current state of hiesho care, and then developed a support model for hiesho care implementation by nursing professionals.
    The study’s aim was to develop and evaluate the support model “Hie Yale” for hiesho care implementation by nursing professionals. The study design is a randomized controlled trial involving nursing professionals. Consequently, the analysis covered 68 participants in the intervention group and 62 participants in the control group. One week after implementation, the knowledge scores of the intervention group were significantly higher than that of the control group in terms of both “knowledge about hiesho” and “awareness of the importance of hiesho care.” The primary outcome of this study was supported.

    The purpose of this study was to verify the effect of warm perineal compresses on the elevation of body temperature and the reduction of perineal pain in women in the early postpartum period . We reviewed the literature concerning warm compresses during early postpartum and visited some postpartum care centers in Taiwan that practiced warm compresses on postpartum women . The results suggested that it needed to consider intervention time based on the wound-healing process, to measure biological indexes as evaluation outcome, or verify the effect of not only perineal pain but also other physical symptoms. Moreover, the quantitative descriptive study was conducted to examine the current state of perineal care in Japan. In this study, the current status and issues of perineal care in Japan were identified to develop a warm perineal compression program that can be performed regardless of facility forms.

    In Japan, the rate of pregnant women who practice antenatal perineal massage was only 15.1%.The aim of this study was to develop and evaluate a smartphone website and a leaflet to support antenatal perineal massage practice for primiparous women.Methods:In a randomised control trial, 161 primiparous women were randomly assigned to a smartphone website group (n = 81) or a leaflet group (n = 80). Data analysis were by per protocol analysis and intention to treat analysis.
    Findings:Of the 161 women participants, 47 in the smartphone website group and 49 in the leaflet group completed all questionnaires. Primary outcome was continuance rate (three times a week over a three week period) of antenatal perineal massage practice. The rates by a per protocol analysis were 51.1% in the smartphone website group and 51.0% in the leaflet group, respectively. There was no significant difference between the groups.
